Just wondering what would cause this error when trying to connect to sim:

That is a very old version of ADE....

I assume you have the sim running and that you have not made any changes to the FSUIPC installation?

Correct and the issue still appears with the 1.51.45 Beta.

Then I guess it must relate to your setup of FSUIPC. Do you have other applications that connect properly without error?

The previous version ran fine. I had not used the program for a while so I updated to the beta. Had a few issues with the install so tried again. Going back to the old version I still had the dialogue box appear. The only thing that uses the FSUIPC is the joystick, CH throttle quad and rudder pedals.

Well I am not sure what to suggest. The beta uses the same connection code as the last production version.

Is your FSX installed in the default location - that is c:\Program Files (x86) - I wonder if User Access Control is getting in the way.... Try running ADE as Administrator and see if that helps.

A Google of FSUIPC Error #12 turns that up as the solution on this post..
